To enroll in health insurance through the Small Business Health Options Program (SHOP), your business or organization must:
- Have a primary business address within Maine
- Have at least one common-law employee
- Have 50 or fewer full-time equivalent (FTE) employees
- Offer coverage through SHOP to all full-time employees
This form will determine your eligibility to purchase SHOP coverage. The Internal Revenue Service determines eligibility for the small business health care tax credit.
